 Hello!

You are receiving this survey because our records indicate that you agreed to be a Foundation Scholarship and Awards Reviewer at one point.

The Foundation has created a new annual charge for our reviewers to stay abreast of best practices. We now require a yearly Confidentiality and Disclosure Agreement Form for our scholarship and award reviewers, plus a short survey. This survey includes the required Annual Confidentiality and Disclosure Agreement Form and a few quick questions to assess your reviewing interests for this year. Completing it should not take more than five minutes.

Thank you in advance for your response and all you do to support excellence in our scholarships and awards program.

We appreciate you!

Sincerely,
The A.C.N.M. Foundation Inc.

PURPOSE:

The Thacher Community Grant supports community-based projects aimed at enhancing overall healthcare outcomes, particularly those demonstrating innovative strategies to meet community needs. This grant is made possible by the Frances T. Thacher Midwifery Leadership Endowment, focusing on improving care for populations facing disparities and limited access to resources while also fostering leadership development among midwives.

Initiatives addressing various community needs, including the care of individuals with physical or mental illness, disabilities, and those in low-resource settings, are preferred. Successful applicants must present a clear demonstration of community needs and a plan to evaluate the project's impact at the community level. Funding is allocated to support the project and the communities it serves directly.

Applicants from diverse backgrounds, including midwives, students, retirees, community groups, advocates, and other health professionals dedicated to serving under-resourced communities, are encouraged to apply.
